Main Duties: 

  • Assist vulnerable adults in making informed decisions about their resources, support, and services to enhance their quality of life and well-being.
  • Engage within Lewisham’s Integrated Health and Social Care model alongside multi-disciplinary teams, including voluntary organizations, to empower independence and personalization.
  • Conduct an initial visit within 24 hours to ensure the safety of the individual and determine the appropriate care pathway: Enablement care team (ECT), supported discharge, permanent care package assessment, or no further intervention.
  • Conduct assessments and re-assessments aligned with the Care Act 2014, utilizing strength-based approaches to develop person-centered support plans and arrange community-based services for individuals and their caregivers.
  • Perform Mental Capacity Assessments and facilitate best interest meetings.
  • Safeguard adults at risk of abuse in accordance with Lewisham Council Safeguarding Policy, the Care Act 2014, and Making Safeguarding Personal.
  • Advocate for self-directed support and direct payments for all individuals and caregivers.
  • Demonstrate knowledge of Continuing Healthcare (CHC) processes and discharge to assess pathways.
  • Exhibit proficiency in report writing, recording, and administrative tasks related to assessment and planning, ensuring accurate documentation on relevant IT systems.
  • Educate clients on support process changes, provider concerns, complaints procedures, and safeguarding awareness.
  • Inform individuals about the Adult Social Care Charging Policy and make appropriate referrals for financial assessments.
  • Communicate effectively and inclusively with individuals who have communication needs or language barriers to ensure equal access to opportunities.

 

Requirements of the Social Worker:

  • Degree or equivalent in social work
  • Current Social Work England registration
  • Significant frontline experience in Adults Services within the UK
