Responsibilities Include:

  • Engaging with individuals in various outreach settings, guiding them towards treatment services and community support.
  • Conducting thorough assessments to understand individual needs and strengths.
  • Delivering tailored care packages, including therapeutic interventions and psychosocial support.
  • Collaborating with healthcare professionals and contributing to multi-disciplinary team meetings.
  • Managing cases with a recovery-focused approach, utilising both one-on-one and group sessions.
  • Providing sensitive communication to service users and their families.
  • Contributing to safeguarding efforts and maintaining safety protocols.
  • Compiling reports, participating in care planning meetings, and ensuring accurate record-keeping.
  • Developing and delivering training sessions for internal and external stakeholders.
  • Actively promoting harm reduction and encouraging access to support services.

Requirements:

  • Relevant degree or diploma in Social Work, Psychology, or Substance Misuse.
  • Demonstrated experience in outreach or similar roles within substance misuse field.
  • Strong knowledge of substance effects, mental health issues, and therapeutic interventions.
  • Ability to work autonomously, manage time effectively, and collaborate with diverse teams.
  • Excellent communication, organisational, and problem-solving skills.
  • Empathetic, non-judgemental approach with a commitment to professional boundaries.
