Responsibilities Include:
- Engaging with individuals in various outreach settings, guiding them towards treatment services and community support.
- Conducting thorough assessments to understand individual needs and strengths.
- Delivering tailored care packages, including therapeutic interventions and psychosocial support.
- Collaborating with healthcare professionals and contributing to multi-disciplinary team meetings.
- Managing cases with a recovery-focused approach, utilising both one-on-one and group sessions.
- Providing sensitive communication to service users and their families.
- Contributing to safeguarding efforts and maintaining safety protocols.
- Compiling reports, participating in care planning meetings, and ensuring accurate record-keeping.
- Developing and delivering training sessions for internal and external stakeholders.
- Actively promoting harm reduction and encouraging access to support services.
Requirements:
- Relevant degree or diploma in Social Work, Psychology, or Substance Misuse.
- Demonstrated experience in outreach or similar roles within substance misuse field.
- Strong knowledge of substance effects, mental health issues, and therapeutic interventions.
- Ability to work autonomously, manage time effectively, and collaborate with diverse teams.
- Excellent communication, organisational, and problem-solving skills.
- Empathetic, non-judgemental approach with a commitment to professional boundaries.
